Work permits are required by the state of Wisconsin for anyone under the age of 18 (except for agricultural or domestic service work), and employers have to have a permit on file prior to the minor beginning work. You can apply for a work permit right through your school office.

➜ Birth certificate / other proof of age
➜ Social Security card
➜ A letter from the employer expressing intent to hire, on employer’s regular letterhead, describing job duties and hours of work
➜ Written parental consent
➜ The $10 application fee (which must be later reimbursed to the student from the employer).
